No. A description of what is going wrong is more useful than a document listing features. If you already have a specification we will read it, and we will tell you where we think it is wrong.
Yes. Send yours and we will review it, or ask and we will send ours. You do not need one in place to describe a problem at a general level, but if you would rather have it first, say so in the form and we will start there.
It depends on scope, and anyone who quotes before understanding the scope is guessing. The budget field on the form is optional and exists so we can suggest the right size of first version. It does not set the price.
